Job Title: Hospice Social Worker
Location: Bridging Life Hospice- Carroll County Facilities
Schedule: Monday–Friday, 8:00 AM – 4:30 PM
Job Summary:
Bridging Life is seeking a compassionate and dedicated Hospice Social Worker to join our interdisciplinary team. In this vital role, you will be responsible for assessing the social, emotional, and environmental needs of patients and their families, creating and implementing individualized care plans, and coordinating with our healthcare team to ensure the highest level of care. This is an opportunity to make a meaningful difference in the lives of individuals at the end of life, while collaborating closely with a team of healthcare professionals to address complex patient needs.
Key Responsibilities:
Patient & Family Assessment: Conduct thorough assessments of patients and families upon admission, identifying social, emotional, and financial needs, as well as support system capabilities.
Care Plan Development: Work in collaboration with physicians, nursing staff, patients, and families to develop personalized care plans that address identified needs and maximize positive outcomes.
Community Resources: Refer patients and families to appropriate community resources to address identified needs, including financial assistance, housing, or specialized services.
Multidisciplinary Collaboration: Actively participate in multidisciplinary team conferences, contributing insights into the social and emotional factors impacting patient care.
Documentation: Maintain accurate and timely documentation in patient records in compliance with regulatory and professional standards.
Case Management: Monitor and evaluate patient progress, making adjustments to care plans as needed based on patient changes and team input.
Crisis Intervention & Support: Provide guidance and intervention in cases of ineffective coping, substance abuse, abuse/neglect, bereavement, and other significant emotional/social challenges.
Ethical Decision-Making: Assist families with advance care planning, end-of-life decisions, and long-term care placement options.
Team Leadership & Professional Development: Serve as a role model to colleagues, providing guidance and support. Engage in continuous learning and professional growth to stay updated on hospice care trends and social work best practices.
Qualifications:
Education: Master's degree in Social Work (MSW) required.
Licensure: Current Maryland LCSW or LMSW license required.
Experience: Previous hospice experience preferred, but not required.
Skills & Abilities:
Strong interpersonal and communication skills.
Ability to assess and address complex emotional, social, and financial factors.
Ability to work effectively in a team-based, collaborative environment.
High level of professionalism, with respect for patient confidentiality and autonomy.
Demonstrated ability to manage multiple tasks and prioritize responsibilities.
